Forms

Academic Standing Certification - Students should contact the Registrar’s Office with requests to certify enrollment and student status. Students in need of a letter (for employers and graduate/professional programs) noting specifically that they are in good academic standing should fill out this form and submit it to the Associate Dean for Academic Affairs (3307 M Street, Suite 202; or fax to 202-687-8954) at least two weeks prior to the date needed by the requestor.

Advanced Professional Certificate (APC) Tracking – Each APC is designed as a planned, cohesive group of courses between the student and their program Associate Dean. While some programs have prescribed courses, others allow for more individualized proposals.  Important:  Students must be admitted formally to an APC and secure the advance permission of the appropriate program Associate Dean(s) for all APC coursework during their first semester of study. A copy of this completed form must be filed with the SCS Academic Affairs Office (3307 M Street, Suite 202, Washington, DC; or via fax to 202-687-8954) before the end of the student's first semester.  APCs will not be awarded retroactively.  [PDF]

Change of Grade – Once course grades have been posted, professors must submit grade changes via this paper form to the Associate Dean of Academic Affairs for review. Grades can only be changed due to an administrative error (such as miscalculation or recording mistake) or when final assignments have been submitted for recorded incompletes. Students cannot submit additional or revised work after a final grade has been recorded for the express purpose of improving that grade. The paper form is available in the Registrar’s Office (G-01 White Gravenor) and the SCS Academic Affairs Office (3307 M Street, Suite 202). [Hard Copy Only]

Graduation Application (Graduate Students Only) – Graduate students in the MALS and MPS programs should apply to graduate within the first two weeks of their final semester by submitting their official graduation application to the SCS Academic Affairs Office (scsgrad@georgetown.edu; 3307 M Street, Suite 202, Washington, DC 20057; or via fax to 202-687-8954). MALS & MPS Application

Graduation Application (Undergraduate Students Only) – BALS students should consult with the BALS office about their application procedures and forms.

Privacy (FERPA) Waiver – Students are protected by the Family Educational Rights and Privacy Act (FERPA), a federal law that limits the information shared about a student with parties (including family members and spouses) other than that student. In order to waive this right, the student should complete this form and submit it to the appropriate office within SCS. [PDF]

Transfer Credit (MPS Only) – With approval of both their program Associate Dean and the Associate Dean of Academic Affairs, MPS students can count up to 6 credits of graduate coursework completed with grades of "B" (3.00) or better at another accredited institution. Credits cannot have been applied to a previous certificate or degree. Requests should be submitted during the student’s first semester of study. Completed forms and transcripts should be submitted to the SCS Academic Affairs Office (3307 M Street, Suite 202, Washington, DC; or via fax to 202-687-8954).  [PDF]

Withdrawal (Add/Drop) – After the official add/drop period has ended, students can only withdraw using a hard copy, paper form.  Students cannot withdraw online, via email, or by calling the Registrar, and they must consult with the Associate Dean of their program to secure approval. If approved by their Associate Dean, the student is solely responsible for hand-carrying this paper form to the Registrar’s Office by the deadline for doing so.  The paper form is available in the Registrar’s Office (G-01 White Gravenor) and the SCS Academic Affairs Office (3307 M Street, Suite 202).
